We are excited to welcome your child to our Faith Formation program at Holy Name of Jesus Church.
Whether your child is returning for a second year of sacramental preparation or enrolling for the first time, we are honored to walk with your family on this important journey of faith.
Before you begin filling out the online enrollment form, please read the following steps needed to finalize your child’s enrollment for the 2026–27 Faith Formation year.
Step 1: Gather Required Documents
Before beginning the online enrollment form, please make sure you have all required documents ready to upload from your mobile device or computer.
You may upload either clear photos of the documents or PDF files. Please make sure all photos are clear and legible.
Step 2: Have Your Holy Name of Jesus Member Number Ready
You will need your Holy Name of Jesus Envelope/Member Number in order to complete the enrollment form.
Families must be active, registered members of Holy Name of Jesus Church for at least three months before enrolling in our Faith Formation program. This is a required field on the enrollment form, and you will not be able to continue without entering this information.
If you do not know your Holy Name of Jesus Envelope/Member Number, please call the church office at 561-683-3555 for assistance.
Please Note: If you are not an active, registered member of Holy Name of Jesus Church, your child must enroll in Faith Formation at the parish where your family is registered.
Families who have registered at Holy Name within the last three months may still submit an enrollment form. However, those enrollments will be set aside and processed after all currently eligible parishioner enrollments have been completed.
Step 3: Submit Payment After Completing the Enrollment Form
After you submit the 2026–27 Faith Formation Enrollment Form, the final step is to complete your enrollment payment.
Please choose one of the following payment options:
1. Zelle — Recommended for the Fastest Processing
Send payment through your banking institution using Zelle to:
hnjpay@myhnj.org
In the Zelle memo section, please include:
- Your child’s full name exactly as written on the enrollment form
- The program your child is enrolling in: HNJ KIDS or HNJ YTH
Example:
Emma Garcia – HNJ KIDS
Payments submitted without this information may delay the processing of your child’s enrollment because we may not be able to match the payment to the correct family.
2. Check
Checks should be made payable to:
Holy Name of Jesus Church
In the memo line, write:
- Your child’s full name exactly as written on the enrollment form
- HNJ KIDS or HNJ YTH
Please place the check in a sealed envelope labeled with your child’s full name and program. Checks may be dropped off at the church office Monday through Friday, from 8:30 AM–3:00 PM or in the after-hours mail slot on the office door.
3. Credit or Debit Card
Credit and debit card payments are accepted by phone or in person at the church office Monday through Friday, from 8:30 AM–3:00 PM.
For phone payments, a receipt may be sent by text message or email. For in-person payments, a printed receipt is available upon request.
4. Cash
Cash payments are accepted in the church office Monday through Friday, from 8:30 AM–3:00 PM.
Please place cash in a sealed envelope clearly labeled with:
- Your child’s full name exactly as written on the enrollment form
- HNJ KIDS or HNJ YTH
